A chronic disease or illness can be described as a long-lasting condition that can be controlled, but not cured completely. Diabetes, high blood pressure, asthma, mental disease, and some heart diseases are just a few examples of chronic diseases. According to statistics, at least 60% of all seniors in America are managing at least two chronic illnesses.
The good thing about chronic illnesses is that, thanks to technological advances, conditions that could only be managed in hospitals, nursing homes, and other institutions a few years ago can now be managed at home through home care services.
The number one goal of home care services is to ensure that people who are ill, disabled, or recuperating from surgery avoid institutionalization for as long as possible, while still receiving the care they need in the comfort of their homes. Health experts believe patients have a higher chance of recovering more quickly and successfully when recovering at home versus in a hospital or other medical facility.
Home care services are available in two forms – non-medical home care and home health care. Home health care is medical in nature and is usually provided by licensed nurses, home health aides, therapists, and other skilled healthcare workers in conjunction with the patient’s healthcare provider. Some of the services that come with home health care may include wound care, medication and injection administration, pain management, ostomy care, intravenous therapy, and others.
On the other hand, non-medical home care is designed to meet the non-medical needs of the patient or client, and is provided by a trained caregiver, in conjunction with a home health care provider, and/or the patient’s healthcare provider. Some of the tasks that come with non-medical home care, also known as in-home care, include light housekeeping, meal preparation, bathing and grooming, toileting, mobility assistance, toileting, companionship, medication reminders, and more.
ROSEWOOD HOME CARE offers personal care services with an emphasis on Chronic Care Management. Alzheimer’s/Dementia Care
For many families, providing care to a loved one who has Alzheimer’s or dementia in the early stages of the illness is often manageable. But with the progression of the illness, a family caregiver may not be in a position to provide the kind of care needed by the patient. This is because, as the illness progresses, so does the needs, and this eventually calls for a greater level of care.
As the primary family caregiver, it is normal to feel overwhelmed by the growing demands of your loved one. But you are not alone –we are here to support you and your loved one through our Alzheimer’s/Dementia Care program.
Just like you, we want your loved one to remain at home for as long as possible. This is because we understand the importance of remaining in a safe, comfortable, and familiar environment for Alzheimer’s/dementia patients. Stroke Care
Surviving a stroke is a good thing, but dealing with the behavioral, physical, and emotional changes after a stroke can be difficult for both the patient and their family. The uncertainty behind not knowing how long it will take for their abilities to improve, or if they will improve at all, can leave everyone feeling confused and helpless.
For many families, providing the kind of care needed by their loved one may not be possible and the thought of moving the patient to a nursing home or assisted living facility can be heartbreaking. This is where our Stroke Care program comes in.
Rather than moving your loved one to a strange environment, we can provide a solution that will have your loved one’s needs catered for in the comfort of their home, leaving the rest of the family free to live their lives without constantly worrying about their loved one.
Our Stroke Care Program is meant to address a wide variety of needs, but because the needs of every stroke survivor are different, the first thing we do is to sit down with the patient’s healthcare provider and family to create a customized care plan for the patient.
All in all, some of the services we can do to support stroke survivors and their families include:
- Assisting with daily rehabilitation exercises as per the instructions of the patient’s healthcare providers.

Diabetes Care
The main goal behind diabetes care is to keep blood glucose levels healthy – not too high and not too low. The good news is that once managed, you or your loved one can live a normal life.
The thing is that, for seniors and other persons who have a condition that affects their mental or physical capabilities, following a diabetes treatment plan is not easy. For these individuals, having someone to take care of their needs around the clock is very important – from establishing healthy eating habits to monitoring their insulin and blood sugar levels.
For families who have a loved one who can’t commit to their diabetes treatment plan for one reason or another, our Diabetes Care Program can help them with that and more. Together with the patient’s healthcare provider and family, we can create a care plan that is tailored to their specific needs
Our program is non-medical in nature and it comes with services like:
- Helping our patients to keep their cholesterol and blood pressure under control through a healthy diet and regular exercise as per the instructions of their healthcare providers.
- Scheduling and accompanying the patient to regular eye care exams.
- Ensuring the patient’s vaccines are up to date. This is because diabetes patients are more prone to illnesses and sicknesses.
- Helping the patient to take care of their teeth and mouth. This is because diabetics are more prone to gum disease and mouth sores.
- Helping the patient to take good care of their feet. Foot care in diabetics is very important as any lesions, cuts, or bruises can get infected real fast.
Other tasks and responsibilities that come with this program include:
- Providing medication reminders.
- Proving minimal medication administration assistance such as handing medications to the patient.
- Providing light housekeeping services.
- Looking out for signs of discomfort in patients and calling for help in case of an emergency.

Arthritis Care
Arthritis is, in many cases, a lifelong condition that can affect persons of all ages, children included. Though the symptoms of arthritis can vary from individual to individual, the most commonly reported symptoms include excruciating pain, joint tenderness, stiffness, swelling, and weak muscles. Commonly affected areas include fingers, wrists, ankles, knees, and hips. The condition can, however, affect any joint in the body.
In some cases, these symptoms and their severity can increase gradually and eventually make it very difficult or even impossible for the victim to carry out everyday activities. Sometimes, even simple activities like feeding and grooming can become totally impossible.
While there’s currently no known way to cure arthritis, the victim’s quality of life can be improved immensely with treatment, lifestyle adjustments, and the support of family and friends. Our Arthritis Care Program revolves around the needs of arthritis patients and their families by providing them with the support they need to cope with the condition.
For the patients, our program supports them in the areas of personal hygiene, nutrition, safety, companionship, support, and encouragement. For family members, it supports them by relieving the burden of care off their shoulders and also by offering emotional support and encouragement.
The specific services offered by this particular program may vary with the individual needs of the patient but generally include:
- Assisting with doctor-recommended exercises. Some exercises have been shown to improve pain, stiffness, and other symptoms of arthritis.
- Providing an arthritis-friendly diet as per the recommendations of the patient’s healthcare provider. A diet rich in vitamin C and Omega-3 fatty acids may help relieve pain according to health experts.
- Helping the patient to keep a healthy body weight through doctor-recommended exercises and diet. Being overweight puts unnecessary strain on body joints and may trigger arthritis or aggravate the symptoms. Losing weight has been reported to help with the symptoms.
Other tasks and responsibilities that come with this program include:
- Providing medication reminders.
- Proving minimal medication administration assistance such as handing medications to the patient.
- Providing light housekeeping services.
- Looking out for signs of discomfort in patients and calling for help in case of an emergency.

Multiple Sclerosis Care
In the simplest terms possible, Multiple Sclerosis can be described as a disease that causes the immune system to eat into the protective cover of the nerves (myelin sheath). When this happens, it can result in nerve damage, which messes up communication between the body and brain.
And no, Multiple Sclerosis is not a disease that affects seniors alone; most cases are actually diagnosed between the ages of 20 and 40. However, the symptoms tend to become a bit severe with age, and most especially when they overlap with the symptoms of other age-related conditions like arthritis.
The symptoms of Multiple Sclerosis (MS) can vary greatly from individual to individual, and also from time to time in the same individual. But all in all, MS can cause the following:
- Lack of coordination
- Weakness
- Extreme fatigue
- Vision problems
- Mood changes
- Cognitive impairment
- Bladder problems
- And others.
For some people, the symptoms of MS are quite mild and don’t interfere in their way of living, but for some, the symptoms can be chronic and severe.
For persons with chronic and severe symptoms, they may need help with their activities of daily living. Depending on the severity of their symptoms, they can get help from family members, but if they need round-the-clock care, in-home care may be a good choice.
If you or someone you love has MS and is in need of round-the-clock care, we can provide all the care they need in their home where they feel most comfortable and appreciated – from helping them with personal hygiene, to ensuring they have eaten a balanced meal.
The services we offer through our Multiple Sclerosis Care Program are dependent on the individual needs of the client, but they generally include the following:
- Assisting with doctor-recommended exercises.
- Providing an MS-friendly diet as per the recommendations of the patient’s healthcare provider. A diet rich in vitamin D, lean meats, fresh fruits and vegetables, whole grains and fatty fish comes highly recommended for these clients.
- Helping the patient to keep a healthy body weight through doctor-recommended exercises and diet.
- Attending to the problems of elimination/incontinence care.
Other tasks and responsibilities that come with this program include:
